Applicants for Cape Girardeau Career and Technology Center’s Physical Therapist Assistant Program must have:

     □     Completed, signed application form for health-related programs.

             Application Deadline is May 1, 2013

     □     Provide (3) professional references (names and addresses: i.e. boss, supervisor, manager, etc.)

     □     Submitted an official high school or GED transcript and official transcripts of all college credit.

     □     Completed prerequisite courses listed below with a grade of “C” or higher:

                 General Biology (min. 3 cr. hrs. with lab)

                 Medical Terminology / Intro to Pathology (3 cr. hrs.)

                 English Composition I (3 cr. hrs.)

                 Intermediate Algebra (or higher) (3 cr. hrs.)

     □     Minimum ACT Composite Score or COMPASS test score (tests must be dated within the past three 
            years)

                  ACT composite score of 19 or higher  or

                 COMPASS scores of 78 Reading, 31 Algebra, and 66 Writing.

                 TESTING DEADLINE IS MAY 1, 2013

    □     Completed a minimum of 24 hours job shadowing experience in approved physical therapy setting.

    □     Completed HOBET (Health Occupations Basic Entrance Test).

             TESTING DEADLINE IS MAY 1, 2013

     □  Applicants for whom English is a second language must show English proficiency by scoring in the
         high level for reading and listening and in the good level for speaking and writing on the TOEFL (Test
         of English as a Foreign Language.  These applicants may postpone taking the TOEFL until they have
         passed the HOBET.

 Each applicant who has completed ALL of the above requirements may interview.

 Qualified applicants will be called for an interview within a month of the May application deadline
